Smart chronic disease management network launched in Xuhui District
A smart chronic disease management network targeting hypertension and diabetes prevention and control through cooperation between doctors in 13 neighborhood health centers, three district-level hospitals and two leading hospitals in Xuhui District was established over the weekend.
The trial consisting of 14,000 residents including 2,400 people with hypertension has kicked off in Jiangnan Community in Xuhui.
The network tries a new method of online and offline management to give patients in-time direction, consultation and guidance on disease treatment and control. Patients can receive all the direction and monitoring from smart phone applications, while doctors in the neighborhood are the direct supervisors.
“To ensure the medical quality, the three district-level hospitals and two leadings hospitals of Zhongshan Hospital and Shanghai No.6 People’s Hospital will give professional supports to improve the quality of health service of neighborhood doctors,” said Dr Fan Jia, president of Zhongshan Hospital. “For complicated condition, patients can be easily transfer from the neighborhood to the superior hospitals.”
Under the new measure, a national chronic disease management commission was also set up in Shanghai to explore better chronic disease prevention and control in China, which is the home of 330 million hypertension patients and over 110 million people with diabetes. If successful, the measure will be promoted to more regions in the nation.
